These SMS Terms and Conditions govern the use of text messaging services provided by Fordland Clinic. By opting in to receive SMS or MMS messages, you agree to the terms outlined below.
SMS and MMS Consent
By providing your mobile phone number to Fordland Clinic, you consent to receive periodic SMS or MMS messages. These messages may include appointment reminders, scheduling updates, clinic notifications, and other communications related to your relationship with the clinic.
Providing Your Telephone Number and Notification Obligations
You confirm that the mobile phone number you provide is accurate and current and that you are the authorized subscriber or owner of the number. If your contact information changes, including your phone number, you agree to notify Fordland Clinic prior to the change by emailing [email protected].
Consent to Receive Automated Communications
By voluntarily providing your phone number, you consent to receive recurring automated and non automated text messages from Fordland Clinic. Consent to receive text messages is not a condition of receiving medical services. Message and data rates may apply.
Opt-Out Instructions
You may opt out of receiving SMS or MMS messages at any time by replying with STOP, END, CANCEL, UNSUBSCRIBE, or QUIT to any message you receive. You may also opt out by emailing [email protected]. You agree to receive one final message confirming your opt-out.

Carrier Participation
Participating carriers in the United States may include AT&T, T-Mobile, Verizon Wireless, Sprint, Boost, U.S. Cellular, MetroPCS, InterOp, Cellcom, C Spire Wireless, Cricket, Virgin Mobile, and others. Carrier participation may vary. Carriers are not responsible for delayed or undelivered messages.
Data Collection and Use
In connection with SMS services, Fordland Clinic may collect your mobile phone number, carrier information, message content, and message timestamps. This information is used solely to provide SMS services and support clinic communications in accordance with applicable privacy laws.
Service Modifications and Termination
Fordland Clinic reserves the right to modify or discontinue SMS services at any time, with or without notice.
Indemnification
You agree to indemnify and hold harmless Fordland Clinic from any claims, liabilities, damages, or expenses, including attorney fees, arising from your provision of a phone number you do not own, failure to notify us of a number change, or violations of the Telephone Consumer Protection Act or applicable state laws.
Participation Requirements
By participating in this service, you confirm that you are at least 18 years of age, have a wireless device capable of two way messaging, are subscribed to a participating carrier, and have an active text messaging plan. Service availability may vary by carrier and device.
Miscellaneous
You represent that you have the authority to agree to these terms and that doing so does not conflict with other agreements. If any provision of these terms is found unenforceable, the remaining terms will remain in effect.
